Services Overview
As a
Provider Group, many of us have been in the
Pensacola-Gulf Breeze Community professionally for
much of our lives. We
take great pride in providing the highest healthcare
possible to our patients, many of which span
generations. As a result, we have one of the
strongest referral networks in the greater
Pensacola area. For inpatient care we admit to
Gulf Breeze Hospital.

We
strive for an open access policy. We have numerous
providers and can usually see people at any time.

in if you feel you need to be seen promptly.
We
are open to see new patients and are available for
physicals, well child exams, respiratory infections,
annual gynecological exams, and provide continuity
care to those patients with chronic medical problems
such as diabetes, heart disease, high cholesterol,
and hypertension. We provide routine care for
individuals with problems in many specialties such
as Cardiology, Pulmonology, Gastroenterology,
Orthopedics, Sports Medicine, Gynecology,
Dermatology, Pediatrics, Psychiatry, Allergy and
Immunology and we perform minor, in office surgery.
If your needs exceed the services we can provide we
will refer you to an appropriate specialist.
Patient Responsibilities
To best serve your
healthcare needs and to make your visit as pleasant
as possible, we have compiled the following
information.

Patients
should arrive 15 minutes prior to their scheduled
appointment. Late arrivals (in excess of 20 minutes
past appointment time) may be asked to reschedule
the missed appointment. We know that your time is
valuable and we will make every effort to honor all
of our commitments.

Your
insurance company requires you to present your
insurance card at each visit. Incomplete or
incorrect insurance information can lead to you
being responsible for your entire fee.

Please bring
an updated list of ALL medications you are taking OR
bottles for these medications with you to each
appointment.
·
Your
insurance policy may not cover the medications,
procedures or lab work your provider feels are
necessary. Every insurance plan is different. Please
take time to educate yourself on the terms of your
insurance policy before your appointment.
·
Scheduled
appointments are preferred. Walk-in patients will be
evaluated and worked into the schedule based on
availability of a health care provider.
·
Questions
regarding your insurance coverage should be directed
to your insurance carrier or your employer's human
resources department.
·
If you have
any forms that need to be completed for school,
work, or insurance purposes, please remember to
bring them to your provider at the time of your
visit. You will be charged an administrative fee for
the completion of a form if it is not presented
during an office visit.
·
We receive
many requests for completion of various types of
documentation that provides financial benefit to our
patients. An administrative fee is charged for
completion of these documents.
·
Please make
sure to schedule a follow up appointment before your
prescription expires. During office visits we
provide enough refills to cover you until your next
scheduled visit. We may charge an administrative
fee for emergency refills.
·
All patients
under 18 years of age must be accompanied by a
parent, legal guardian, or have signed
parental/legal guardian consent for treatment.
Emergencies
·
If you
find yourself in a medical emergency, please call
911 or go to the nearest Emergency Department.
After Hours, Weekends, Holidays
·
We have a
physician on call 24 hours a day. The function of
the on call physician is to respond to hospital
admissions from the emergency department at Gulf
Breeze Hospital and for caring for our patients who
are admitted to Gulf Breeze Hospital. The on call
physician is not on call to schedule appointments,
prescribe medications or authorize refills,
including refills for pain medications and
antibiotics.

After hours,
weekends, and holidays calls are directed to our
answering service. The service will take your name,
phone number, and a brief description of the
problem. They will notify the on-call provider who
will provide feedback. |
